Now that you are scheduled for a New Patient Exam, what's the next step?

Time to download and complete the clinic forms. The links are down below.

Give yourself about 20-30 minutes to complete the paper work. I recommend doing this ahead of your appointment so you have plenty of time since there will be a lot of information and questions.

If you are unable to complete the forms at home for what ever reason, please contact me and let me know. I will have you either swing by and pick up the forms ahead of time or arrive 20 minutes early to your appointment to complete. We want to avoid you filling paperwork during your appointment time.

Must importantly, please bring your clinic forms to your appointment.

What you need to know before your first treatment...

If possible, please wear comfortable clothes that are loose fitting like sweatpants. Please avoid tight skinny jeans or leggings.

Definitely come well hydrated. Please avoid drinking caffeine drinks just prior to your treatment. The goal is to feel relaxed and not jittery.

Please make you are not fasting and have eaten something within a few hours.
​

Use the restroom before your appointment time.
​

Please turn your phone ringer off or turn phone on airplane mode.

Please download all 3 forms, print, and complete. Allow yourself 30 minutes to fill out and please do so before your appointment. And don't forget to bring the forms to your appointment.
Below is the HIPAA Guidelines to read. You do not have to print this out, unless you would like it for your records. Signature is required from the above form called "Notice of Privacy". Thank you!
Telehealth: This form is for virtual consults only. Please be sure to complete the New Patient Forms as well. You do NOT need to complete the COVID Form since you will not be coming in person to the clinic.
